Assisted Living Facility Forced to Shut Down Due to Care Deficiencies

Officials have forced an assisted living facility to close after state investigators say the facility failed to provide adequate care to patients. The director has 20 days to close the faicility.

The State Agency for Health Care Administration said the facility permitted two patients to wander away and did not act adequately to prevent a man with schizophrenia from forcibly raping another patient who suffered from dementia.
