Investigation into Death at Assisted Living Facility

An elderly resident died days later after she received head lacerations and multiple bruises to her leg, hip and spine. According to facility reports which included a few discrepancies, the resident was discovered either on the floor or lying on the sofa with blood present.

After the resident was found, family members were called but not informed of the complete details of her injuries. One employee stated that HIPPA prevented them from disclosing information although the family was the HIPPA respondent and it was documented on record.

The resident was transferred to the hospital and family members were told to meet her there and it was then that the family learned the full extent of her injuries. The family paid extra money to the assisted living facility to ensure that the resident was in bed at a certain time. Family members say that the staff was neglectful in their duties.
